I hereby give permission for images of my child, captured during the alpaca activities, through video, phone camera and digital camera to be used solely for the purpose of Annabelle’s Alpacas, including the Facebook and Instagram page. Last names of minors will not be given or posted on the internet or website.
TERMS AND CONDITIONS
IMPORTANT INFORMATION - Parent/Guardian Responsibilities:
Alpacas are gentle animals and respond well to interaction with adults and children alike. They are generally quite nervous so we discourage people from running up to them or approaching them from behind.
Annabelle’s Alpacas take all reasonable care to ensure that its events are fun and safe.
Gentle as they are, there is always an element of risk with any animal. As such we you must agree that any injury caused to any child, vulnerable person or adult in attendance, however unlikely, is the sole responsibility of the parent/guardian/carer/organiser/others in attendance. This includes any medical issue that could arise which needs attention.
As far as is permitted under Local Law, I assume the above risks and accept responsibility for any injury sustained by my child/vulnerable person/other persons. I further discharge and hold harmless Annabelle’s Alpaca including its owners colleagues from any liability arising from any injury to my child or other persons or property caused by our participation in any Annabelle’s Alpacas event.
I understand that by signing this form I am waiving valuable legal rights.
Event Cancellations:
All cancellations must be made within 1 week of attendance at the event
I understand and agree that I will not receive any refund or credit for any session cancelled less than a week (7 days) before it is due to go ahead.
If cancellation of an event is due to failure on the part of Annabelle’s Alpacas I will be entitled to a reasonable refund of the charge for that event.
All parents/guardians/organisers will inform the organiser in advance of the session if any illness, cold or covid symptoms and a decision will be made as to whether it is safe to continue the event. The event coordinator will be informed if there is a positive LFT or PCR test of anyone in attendance.
Confirmation
BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
